California is the largest physician market in the country and the hardest one to buy in. Two facts shape every file here, and most of what you will read online has neither.

Are California's physician loan-repayment programmes open?

No. Both were closed to new applications when we checked on 2026-10-05, and neither reopens this year.

How big is California's physician market, really?

Second in the country by headcount, and below average per head — a combination that explains a lot about the state.

From the ACGME Data Resource Book, academic year 2024-2025. New York, with roughly half California's population, trains more residents outright — 102.56 per 100,000, about two and a half times California's rate.

So California's training capacity is thinner than its size suggests, and it is concentrated in the most expensive housing markets in the country. That is the squeeze this site is about.

Why a physician loan matters more in California than almost anywhere

Because of where conventional financing stops. A physician structure reaches $2M with no monthly PMI. California's conforming limits do not come close in most of the state.

Between the ceiling and $2M there is $750,875 of room where a California physician is in jumbo territory on conventional financing and still inside the physician programme. In the 41 baseline counties that gap opens at $832,750 and runs $1,167,250 wide.

What happens to the training debt

This is usually what decides whether the file works. A physician programme can qualify you on your documented income-driven payment. A conventional loan instead counts 1% of the outstanding balance (Fannie Mae B3-6-05), and FHA counts 0.5% of a zero-payment balance (HUD 4000.1).

On a $300,000 training balance that is the difference between a documented payment and a $3,000 monthly phantom debt. In a market where the typical Bay Area home is over $1.1M, it is the whole ballgame. How California underwriting handles it.

Who the programme covers

Eligible degrees under the Sequoia Medical Professionals Program guide (v1.1, effective March 2, 2026) include MD, DO, DDS, DMD, DPM, OD, PharmD, DVM and CRNA. Residents and fellows qualify.
